This dashboard details the type of device; type of device by residual harm to the patient; device defect, failure, or user error; device defect, failure, or user error by residual harm to the patient; type of health information technology (HIT) device in HIT-related report; and type of HIT device in HIT-related report by residual harm to the patient. Selecting one of the options in the top table below will display a related figure and table. This information can also be downloaded as an Excel file from the links in the Additional Resources box.

The Network for Patient Safety Databases (NPSD) does not contain a representative sample of patient safety concerns and cannot be used to calculate the actual incidence or prevalence of patient safety concerns. The reporting of patient safety concerns to the NPSD is voluntary as is the reporting to Patient Safety Organizations (PSOs) by providers. The NPSD is a summary of certain elements in Hospital Common Formats Events Reports for specific types of patient safety concerns that have been submitted voluntarily by a portion of the AHRQ-listed PSOs.
