The Network for Patient Safety Databases (NPSD) does not contain a representative sample of patient safety concerns and cannot be used to calculate the actual incidence or prevalence of patient safety concerns. The reporting of patient safety concerns to the NPSD is voluntary as is the reporting to Patient Safety Organizations (PSOs) by providers. The NPSD is a summary of certain elements in Hospital Common Formats Events Reports for specific types of patient safety concerns that have been submitted voluntarily by a portion of the AHRQ-listed PSOs.
