SOPS Nursing Home Survey Database
ÂÜÀòÊÓÆµhas established the Surveys on Patient Safety Culture® (³§°¿±Ê³§Â®) Nursing Home Survey Database as a central repository for survey data from nursing homes that have administered the ÂÜÀòÊÓÆµpatient safety culture survey instrument and choose to submit their survey data to the SOPS Nursing Home Survey Database. The Database serves as an important resource for patient safety culture improvement.
Participation is free and open to all SOPS Nursing Home Survey users, provided the questionnaires are administered in a manner consistent with SOPS guidance and survey data are submitted according to SOPS specifications. Participating organizations receive a customized feedback report with their own results compared to the Database results. The SOPS Nursing Home Survey Database contains data voluntarily submitted by participating nursing homes and is not representative of all U.S. nursing homes.

- Workplace Safety Supplemental Items for nursing homes.
- Using the Data for Research—Provides information and application instructions for researchers interested in obtaining de-identified data files from the database to conduct research.
SOPS Nursing Home Survey 2.0 Pilot Test Results
Pilot Test Results from the 2025 SOPS Nursing Home Survey Version 2. 0 (PDF, 621 KB); Appendixes (PDF, 587 KB)—Results for the SOPS Nursing Home Survey Version 2.0 includes data from 27 U.S. nursing homes with over 1,300 respondents who participated in a pilot study in September – November 2024.
SOPS Nursing Home Survey 1.0 Database Report Results
2025 Nursing Home Survey Database Report (PDF, 2.7 MB); Executive Summary (PDF, 1.2 MB); Appendixes (PDF, 990 KB)—Presents data from 107 U.S. nursing homes and 4,411 nursing home staff respondents that administered the survey and submitted data. The report consists of a narrative description of the findings and two appendixes, presenting data by nursing home characteristics and staff respondent characteristics for the database nursing homes.

2025 Updated Results for the SOPS Workplace Safety Supplemental Items for Nursing Homes (PDF, 1.3 MB; Executive Summary, PDF, 870 KB); Appendixes, PDF, 905 KB)—Presents updated results for the Nursing Home Workplace Safety Supplemental Items, including data from the 2023 Workplace Safety pilot test and the 2025 Nursing Home Survey Version 2.0 pilot test for a total of 3,683 providers and staff in 72 U.S. nursing homes.
